Important Information

> Please take note of the cut-off dates for entries & accommodation registrations.

> If you don't require club booked accommodation, complete the  below registration form for your carnival entry.

>Accommodation prices quoted are per person.

>Full payment for your chosen accommodation package/s is required at completion of the form.
>Accommodation packages cannot be altered  - members must pay in full for their relevant package whether they are staying the full number of nights or not 

>For reimbursement from your fundraising please email Karen (surfaccounts@kurrawasurf.com.au) a copy of your accommodation receipt along with your bank details 

> Reimbursements from fundraising proceeds will be made from Thursday, 8th June 2023 for Ocean Roar 

> Reimbursements from fundraising proceeds will be made from Thursday, 13th July 2023 for Australian Titles

> If you submit an entry form and do not compete at a carnival without notifying your coach of your intentions you will be invoiced for the full amount of your entry fee.
